Nurse Case Managers

Description:	GUILDNET our rapidly growing innovative long-term managed care program for the visually impaired/blind population throughout the New York City region, is now offering excellent career and growth opportunities. We are currently looking for candidates with a successful track record of thriving in a fastpaced and challenging Managed Care and/or Home Care environment, to fill the role of Nurse Case Manager. In this position you will provide telephonic case management of long-term health care services allowing plan members to remain in the community by collaborating with health care providers and developing personalized plans of care.

Requirements:	• BSN or relevant Baccalaureate degree preferred. • Candidate must have a valid NYS RN license. • 2 years experience in Case Management and/or Home Care preferred. • Computer literacy.

Speech Therapist

Description:	The Guild School now has an immediate opportunity for a Certified Speech Pathologist to work as part of an interdisciplinary team providing services to school age students who are blind, visually impaired or deaf-blind and have additional disabilities.

Requirements:	·MA degree in Speech and Language Pathology. ·NY/SED licensure as a Teacher of Speech and Hearing Handicapped (TSHH) ·Clinical certification; CCC or CFY ·Knowledge of augmentative communication/adaptive technology. ·Bi-lingual Spanish a plus

HOME BASED THERAPISTS

Description:	HOME BASED THERAPISTS-Flexible Per Diem Hours (Physical, Speech and Occupational Therapists) The Jewish Guild for the Blind provides the widest array of health-related services for people who are blind or visually impaired and mutli-dsiabled. The Early Intervention program is expanding to include home based services for children with visual impairments. Join our current team of home based professionals in providing home based services to children 1-3 years of age in the Bronx, Brooklyn, Queens or Manhattan.
