The Guild offers comprehensive case management for persons with HIV/AIDS who are visually impaired or blind. Whether a client's vision loss is sudden, from an opportunistic infection, or gradual, from an earlier phase in life, special vision-related rehabilitation needs may be present.
Our comprehensive case management takes into account the unique health and personal necessities of persons who are vision-impaired with HIV/AIDS. The AIDS Case Management Program is one of many mental health services provided by The Guild, where extensive rehabilitation services provide clients with the widest continuum of care.
These services are available in all five boroughs of New York City and are supported, in part, by Ryan White Title I funds.
What We Offer
- Coordination and linkage between the client and all the health and support services needed to maintain the person's ability to remain in the community of his or her choice.
- Collaboration with the primary care physician to assist the client in following through with necessary medical appointments and health care needs.
- Continuity of care for the client by helping to retain established service providers.
- Identification of and advocacy for appropriate community resources in the care network, such as: vision rehabilitation, specialized psychiatric care, and educational, social, housing, recreational and legal services.
- Coordination of benefits and entitlements, as well as help in obtaining them.
- Support for the client's caregivers, family and/or significant other.
