The Guild's Maxine and John M. Bendheim Center for Diabetes Care offers comprehensive Diabetes health care to persons who have impaired vision, or who may be at risk for vision loss as a result of Diabetes. Each patient receives a customized plan of care that addresses all of his or her health care needs. This comprehensive approach allows providers to collaborate in a patient-centered environment and thereby facilitate continuity of care.

More than eight percent of the U.S. population has Diabetes, and almost half do not know they have it, according to data from the American Diabetes Association and the New York State Department of Health. The Guild's Center for Diabetes Care targets people who are most at risk for the disease. Through outreach and education, our Center for Diabetes Care works to intervene at the earliest stages, thus reducing the risk of complications such as blindness, kidney disease and amputation.

The Guild's Center for Diabetes Care specializes in addressing the unique needs of blind and visually impaired people. It has particular expertise in managing patients who require special attention and services, while working collaboratively with the patient's own care providers.

What We Offer

Diabetes Self-Management Education Program - This intensive program teaches people with Diabetes how to manage their disease in order to minimize complications, preserve remaining vision and improve their quality of life. Our Diabetes Self-Management Education Program has been recognized by the American Diabetes Association in accordance with the National Standards for Diabetes Self-Management Education Programs.
