Eligibility Requirements for GuildNet Enrollment
- Age 18 or older
- A resident of Manhattan, Queens, Bronx, Brooklyn, Nassau or Suffolk
- Eligible for Medicaid
- Eligible for nursing home level of care (as of the time of enrollment) determined by using the New York State patient assessment instrument
- Capable of living in or returning to the community without jeopardy to enrollee's health or safety
- Expected to require the long-term care services offered by the program for at least 120 days
This program focuses on visually impaired and blind individuals. Immediate family members and caregivers of blind or visually impaired persons are also encouraged to enroll.
